How to Find & Complete Your Forms
To ensure a smooth and timely check-in at our office, please complete all required paperwork prior to your visit. Here is how you can access your child’s forms.
Look for a welcome email or an electronic check-in link sent directly by our office prior to your child's appointment.
Log in to your patient portal account, and navigate to the "Forms" or "Documents" section.
If you do not have an active portal account, please call our office front desk, and a member of our team will gladly email or text you the digital intake paperwork.
New Patients
Getting Started at My Pediatrics PC.
Account Activation: After scheduling your child’s first visit, check your inbox for a portal activation email from our office.
Pre-Visit Intake: Complete all health history, insurance details, and consent forms in the portal at least 24 hours before your appointment.
What to Bring: Please remember to bring a valid photo ID, your insurance card, and your child’s up-to-date immunization records to the office.
